Re: Validation for Customs Power of Attorney

The below mentioned documentation is the only documentation that U.S. Customs will

accept in order to Validate IRS Number and/or Social Security Number.

1) IRS Form 1120 – US Corporate Tax Return

2) IRS Form 147C – letter from IRS assigning number

3) IRS Form 1040 – individual tax return

4) IRS Form 941 – employer’s record of tax liability

5) IRS Form SS4 – application for EIN

6) IRS Form 1065 – partnership tax return

7) IRS Form 8109 – federal tax deposit coupon

8) Social Security Card and Pictured I.D.
